Spay & Neuter Contract
Please take a moment to answer a few questions about yourself and your family. We ask that you review your
responses to insure that you've answered all the questions, any applications with blanks will be rejected. If
the question does not apply to you, please respond N/A.
Will this Vet require an information release form in order for Tip of Tex K-9 Rescue to
verify information?

Please list the Owner's name on medical records  if different from Applicant.
Please list the Name of Dog on medical records  if different from our records.
Please keep a copy of this contract for your personal records.
Please read the following terms carefully. If you agree, E-sign your application and submit.
    I certify that the information provided on this form is true & correct. I understand
    that proper food and veterinarian care can be costly and I am financially and
    physically able to care for this animal.

    Adopting Party agrees to neuter/spay the puppy before it reaches one year of age.
    Adopting Party agrees to supply Tip of Tex K-9 Rescue with a veterinarian's letter by
    certified mail as proof of a neuter/spay. If Adopting Party fails to neuter/spay this
    puppy and provide proof to the Tip of Tex K-9 Rescue by one year of age, rights and
    ownership of the dog will automatically terminate and revert back to Tip of Tex K-9
    Rescue who may immediately assume possession of the dog.

    Adopting Party acknowledges and agrees that this puppy will be registered with their
    local city or county. In the event that the Adopting Party does not have the puppy
    properly neutered/spayed, any offspring issuing from this puppy are not by any means
    the responsibility of Tip of Tex K-9 Rescue.



Typing your name below will serve as legal signature. By signing below, I am attesting to the
truthfulness of my answers. Falsification of any of the above information will be grounds for
denying the adoption of rescue dog and possible removal of dog adopted. Applicant must be 21
years of age or older. Tip of Tex K-9 Rescue reserves the right to refuse any applicant.
